Making patient blood management the new norm(al) as experienced by implementors in diverse countries
Despite compelling evidence and ongoing WHO policy drive, practical guidance for healthcare providers and national authorities and clinical guidelines and recommendations across numerous specialties and national health systems, implementation of Patient Blood Management is still far behind the expectations for good and safe clinical practices.
Unless translated into the daily routine and organizational culture, evidence is of limited value. To bridge the gap and effectuate the necessary culture change, it is essential to understand the drivers and barriers for Patient Blood Management as well as the stakeholders’ roles and responsibilities.

Intravenous iron: a framework for changing the management of iron deficiency
Administering iron intravenously is clearly the fastest way of correcting an iron deficit, and its efficacy is unquestionable.
In addition to superior outcomes with intravenous iron in patients with heart failure, chronic kidney disease, inflammatory bowel disease, and pregnancy, intravenous iron should be a frontline therapy in abnormal uterine bleeding.
With the advent of new intravenous iron formulations that allow complete replacement dosing in just 15–60 min and with more favourable safety profiles, physicians should be motivated to consider giving a higher priority for the use of intravenous iron as a treatment for iron deficiency.
